Bill Gassen
2027 Board Chair
President & CEO | Sanford Health
Last verified November 4, 2026
Affiliations
- •Becomes 2027 AHA Board Chair on January 1, 2027. He was named Chair-Elect Designate in July 2025 and served as Chair-Elect in 2026.
- •President & CEO of Sanford Health since November 2020. After the January 1, 2025 Marshfield Clinic Health System combination and the September 1, 2026 merger with Minneapolis-based North Memorial Health, Sanford spans the Dakotas, Minnesota, Wisconsin, Iowa and Nebraska with roughly 56,000+ employees.
- •Appointed in March 2026 as one of 18 members of the HHS/CMS Healthcare Advisory Committee, which advises the HHS Secretary and CMS Administrator on Medicare, Medicaid and CHIP.
- •Chairs the Coalition to Strengthen America's Healthcare, the AHA-founded 501(c)(4) advocacy and ad campaign group that opposes Medicare and Medicaid cuts.
- •Has sat on the board of Oscar Health, Inc. (NYSE: OSCR), a publicly traded health insurer, since December 2022.
- •Board member of Medical Alley and the Greater Sioux Falls Chamber of Commerce. He is a lawyer who was Sanford corporate counsel (from 2012) and chief human resources officer before becoming CEO.
Financial / Compensation
- •His Form 990 compensation rose to about $5.0 million in FY2024 ($4,414,119 reportable plus $585,241 other), up from about $3.26 million in FY2023 ($2,821,352 plus $439,680), a 53% one-year jump. FY2022 was about $3.06 million and FY2021 about $3.35 million.
- •Schedule J breakdown (FY2024, the Sanford Group Return): base salary $2.33M, bonus and incentive $1.62M, other reportable $455,266, retirement and deferred $584,349, nontaxable benefits $892; total $5.00M. Bonus and incentive pay was 32% of the total.
- •The Sanford parent return (EIN 45-3791176) reported $6.11 billion in revenue and $281 million in net income for FY2024, after $5.74 billion in revenue and $365 million in net income in FY2023.
- •Systemwide audited FY2025 results, the first year including Marshfield, showed $11.7 billion in operating revenue but only $210.9 million in operating income, a 1.8% margin, down from 5.2% in 2024. Net income still reached $1.1 billion on investment and nonoperating gains.
- •His 53% raise in FY2024 came the year before the system's operating margin fell by more than two-thirds.
- •Former CEO Kelby Krabbenhoft collected about $49.5 million on his November 2020 exit ($15 million severance, $29.4 million retirement payouts, $5.1 million salary). That was part of about $65 million paid to six departing executives in one year.
- •As an Oscar Health director, Gassen receives board compensation from an insurer whose rates and networks depend on negotiations with hospital systems like his own.
Lobbying and Political
- •Senate LDA filings under the client name Sanford Health show $720,000 in outside-firm lobbying in 2025 (Bockorny Group $400,000; Chamber Hill Strategies $320,000) and $540,000 for the first three quarters of 2026. OpenSecrets, which can aggregate affiliates differently, previously put Sanford at $560,000 (2020), $910,000 (2022) and $960,000 (2024).
- •Marshfield Clinic Health System, now part of Sanford, files its own LDA reports: $240,000 in 2025 and $180,000 through Q3 2026. Combined Sanford-family federal lobbying was about $960,000 in 2025.
- •Sanford's 2025-2026 filings list Medicare reimbursement rates, Medicaid reform, budget reconciliation, across-the-board spending cuts, federal hospital funding, rural health and immigration as lobbying issues.
- •Sanford-affiliated PACs and individuals contributed $246,174 in the 2024 cycle, per OpenSecrets.
- •As chair of the Coalition to Strengthen America's Healthcare, he heads a 501(c)(4) that is not required to disclose donors. In 2025 it ran national TV and digital ads and generated more than 575,000 letters to Congress against Medicare and Medicaid cuts.
- •As 2027 chair he takes over an AHA that reported $25.47 million in federal lobbying in 2025, then cut spending 24% in the first three quarters of 2026.
Revolving Door
- •In March 2026 he joined the HHS/CMS Healthcare Advisory Committee while serving as AHA Chair-Elect, so the incoming head of the hospital lobby is formally advising the agency that sets hospital payment rates.
- •At the same time he chairs the AHA's 501(c)(4) issue-advocacy arm and sits on a publicly traded insurer's board (Oscar Health).
- •From January 2027 he will hold the AHA chair and a federal advisory seat at the same time, an unusual concentration of industry, lobbying and regulatory-advisory roles in one person.
Home System Record
- •Sanford Health's charity care at cost was $89.1M (latest hospital tax returns).
- •Sanford Health has 458 340B contract pharmacy arrangements registered with HRSA (OPAIS registry, Sept. 29, 2026).
- •Sanford completed its merger with North Memorial Health (Robbinsdale and Maple Grove hospitals, Minneapolis suburbs) on September 1, 2026. Minnesota AG Keith Ellison approved it under a 10-year oversight agreement that includes a $600 million capital commitment, the first major test of the merger-review law Minnesota passed after Sanford's failed Fairview bid.
- •Sanford's merger record: it completed MeritCare (2009); the FTC and North Dakota AG blocked Mid Dakota Clinic (2017), with the court accepting that the deal would create 75-85% shares in Bismarck-Mandan physician services; talks with UnityPoint (2019) and Intermountain (2020) collapsed; and Fairview/M Health (2023) was abandoned after a Minnesota AG investigation.
- •The Marshfield Clinic Health System combination closed January 1, 2025, taking Sanford into Wisconsin. Prairie Lakes Healthcare (Watertown, SD) and the Lewis Drug retail pharmacy chain were also acquired.
- •In April 2025 Sanford laid off about 96 leadership and administrative employees as part of the Marshfield integration, while pledging $500 million of investment in the Marshfield region over five years.
- •The FTC found Sanford and Mid Dakota Clinic were 'each other's closest rivals' and that the merger would likely raise reimbursement rates and premiums.
- •Sanford participates in 340B across eligible hospitals, a Sanford pharmacy executive was elected to the 340B Health board in 2021, and Sanford considered but declined converting critical access hospitals to rural emergency hospitals partly because of the potential loss of 340B discounts. The Lewis Drug acquisition gives it a retail pharmacy network to run contract-pharmacy and in-house dispensing.
- •Lown Institute's 2025 fair-share report does not cover the Dakotas, so no hospital-level figure exists for Sanford's core market. The states Sanford has expanded into run large statewide deficits: Minnesota $487 million, Wisconsin $493 million and Iowa $162 million a year (2020-2022 averages).
Litigation and Scrutiny
- •Hanson v. Sanford Health (D. Minn., No. 25-cv-03129): a proposed class action alleging Sanford embedded Meta and other tracking pixels on its website and disclosed patients' data. On September 4, 2026, Chief Judge Patrick Schiltz denied dismissal of the federal Electronic Communications Privacy Act and Minnesota deceptive trade practices claims, so the case proceeds.
- •Former Sanford surgical oncologist Dr. Sabha Ganai alleges she was fired in retaliation for raising patient-safety concerns. The suit was filed in Cass County, N.D., in July 2024 under the state whistleblower law and has moved to federal court with discrimination and FMLA claims. It was pending as of March 2026.
- •Sanford Health, Sanford Medical Center and Sanford Clinic paid $20.25 million to settle False Claims Act allegations of Anti-Kickback violations and medically unnecessary spinal surgeries tied to neurosurgeon Wilson Asfora's physician-owned distributorship. Whistleblowers received $3.4 million, and Sanford entered a Corporate Integrity Agreement with HHS-OIG.
- •The whistleblower complaint alleged that Sanford leadership got 'numerous warnings' about the scheme. Sanford had earlier paid $625,000 to resolve kickback allegations involving the same surgeon.
- •FTC and State of North Dakota v. Sanford Health (2017): a federal court enjoined the Mid Dakota Clinic acquisition, and the Eighth Circuit affirmed.
- •Minnesota AG Keith Ellison investigated the proposed Sanford-Fairview merger in 2022-2023 under antitrust and nonprofit-asset law, received 3,500+ public comments and asked that closing be delayed. The deal was abandoned in 2023.
Red Flags
Concurrent capture: from January 2027 he holds the AHA chair, chairs the AHA's dark-money-capable 501(c)(4), sits on a federal CMS/HHS advisory committee and holds an insurer board seat.
His pay rose 53% in one year, to about $5.0 million in FY2024, and the next year Sanford's operating margin fell from 5.2% to 1.8%.
Serial consolidation: one deal blocked by the FTC as a near-monopoly, one abandoned under state AG scrutiny that prompted a new merger-review law, then Marshfield (2025) and North Memorial (2026), which pushed Sanford into the Twin Cities under a 10-year AG oversight agreement.
The $20.25 million False Claims Act settlement and Corporate Integrity Agreement followed alleged repeated warnings to leadership about a surgeon's kickback scheme, after Sanford had already paid to settle similar allegations about the same surgeon.
A federal judge let a patient-privacy class action proceed in September 2026 over Sanford's alleged sharing of patient web data with Meta.
About $65 million in executive exit payouts in one year at a nonprofit, including about $49.5 million to a CEO forced out over his COVID-era conduct.
Sanford kept its namesake and donor relationship with T. Denny Sanford, whose fortune came from subprime credit cards, after ProPublica reported in 2020 that he was under investigation. No charges were filed.
Pattern Summary
Bill Gassen is a corporate lawyer turned CEO who has grown Sanford past $11.7 billion in revenue by absorbing Marshfield in 2025 and North Memorial in 2026, after regulators blocked or derailed two earlier deals. His own pay rose 53% to about $5 million in FY2024, the year before Sanford's operating margin collapsed to 1.8%. In January 2027 he becomes AHA chair while also sitting on HHS/CMS's federal advisory committee, chairing the AHA's 501(c)(4) and serving on an insurer's board. That puts the hospital lobby's top officer inside the agency that sets its rates.
